No. Many of our members are uninsured or self-pay and use Casa De Salud as their primary source of care. However, we do recommend having some form of coverage (or a health-sharing plan) for situations that require hospitalization, surgery, specialist care, or imaging —services outside the scope of primary care.

We do not bill insurance for any of our services. Your monthly membership covers all primary care visits. However, we can provide a superbill (an itemized receipt) that you may submit to your insurance company for potential reimbursement, depending on your plan.

Traditional Medicare patients are welcome at Casa De Salud on a single-visit (fee-for-service) basis. Federal law prohibits traditional Medicare patients from enrolling in a DPC membership. We do not accept any Medicare Advantage plans. Please note: we have a limited number of Medicare spots available — contact us to check current availability.

We do not accept Medi-Cal or Medicaid however patients can still become members for a monthly fee.

Yes. Our doctors are trained in family medicine and see patients of all ages. Currently we are accepting children ages 12 and older. The same membership fee applies — contact us to enroll your family.

Direct Primary Care is a membership-based model of primary care where you pay a flat monthly fee directly to your doctor — no insurance middleman, no copays, no surprise bills. At Casa De Salud, that means easy access to your physician for one transparent monthly rate.

In a traditional practice, doctors see 20–30 patients a day because they depend on insurance reimbursements. That means rushed appointments and long waits. At Casa De Salud, our membership model allows us to keep our patient panel small so we can offer same-week appointments, longer visits, and direct access to your doctor by phone or video.

No. Concierge practices typically charge high annual fees on top of billing insurance. Casa De Salud is a Direct Primary Care clinic — we charge one flat monthly fee and do not bill insurance for our services. Our model is designed to be affordable and accessible.

Currently our fee is $125 per month. There is also a one-time $50 enrollment fee when you join. No copays. No deductibles. No surprise bills.

Your membership includes: easy access to in-person and telehealth visits, same-week appointments, preventive and wellness care, chronic disease management, acute sick visits, mental health and lifestyle medicine support, women's health services, access to your patient portal, discounted lab rates (50–90% off), and care coordination with specialists, imaging, and hospitals.

No. Membership is month-to-month. You can cancel with 30 days' notice at any time.

Yes — Casa De Salud is HSA and FSA eligible. We do not bill your HSA directly, but we are happy to provide a superbill for potential reimbursement.

Yes, we offer single visits. We cannot guarantee ongoing care if you are a seen for a single visit.

We provide comprehensive primary care for patients of all ages, including: preventive care and annual wellness exams, diabetes, hypertension, high cholesterol, and thyroid disorders, respiratory infections, UTIs, and other acute illnesses, anxiety, depression, and stress-related concerns, women's health including reproductive health and preventive screenings, functional medicine consultations (root-cause approach), and Osteopathic Manipulative Treatment (OMT) for pain and alignment.

Yes. Telehealth visits are included in your membership. You can connect with your doctor from home for many types of visits.

We offer same-week appointments. Because our patient panel is intentionally small, you won't wait weeks to be seen.

Yes. As a member, you have access to lab work at 50–90% off standard retail pricing through our negotiated rates. We can also help identify cost-effective options for medications.

Yes. Dr. Blair Cushing, DO offers Functional Medicine consultations and Osteopathic Manipulative Treatment (OMT), a hands-on therapy that improves alignment and supports natural healing. OMT is billed separately by time.
